Clotrimazole is an antifungal that belongs to the group of Imidazoles, it acts against dermatophytes, fungi and yeasts. It interferes in the synthesis and permeability of fungal cell membranes through the inhibition of 1-4 alpha-demethylase (associated with the cytochrome P450 system), one of the enzymes that catalyzes the conversion of Lanosterol into Ergosterol, the most abundant lipid in the membrane in fungi. Clotrimazole, by inhibiting the synthesis of Ergosterol, produces changes in the lipid composition of the fungal cell membrane, this structural change alters cell permeability and as a result osmotic disruption or inhibition of fungal cell growth.
Under proper test conditions, minimum inhibitory concentrations (MICs) for dermatophytes, yeasts and molds are at levels of 0.0642-4 mg / L. The type of action is primarily fungistatic, although fungicidal effects appear in dermatophytes and Candida spp from concentrations greater than 10-20 mg / L.
Tests carried out with the local administration of Clotrimazole showed that it penetrates well into the various layers of the skin; During the following 6 hours of application, the MIC values found in in vitro tests for the most important mycoses are reached in the deepest layers of the epidermis.
The absorption of Clotrimazole vaginally is almost nil, approximately 3.0-10% remaining for 3 days after its application in fungicidal concentrations, the plasma concentrations are very small. It is metabolized in the liver and excreted in feces and urine as an inactive metabolite.
